Registered and Licensed Nurses

A nurse will visit you at pre-scheduled times. They will make special efforts to include both you and your family in your care plan. Our nurses provide treatments such as:

Dressing changes & Wound care
Foley Catheter care and insertion (Urinary management)
IV, IM, & SubQ medication administration
Assessing, monitoring, and supervision of patient’s health conditions
Medication instruction and compliance
Home Exercises demonstration and monitoring
Care coordination and communication with physicians/pharmacy
Bowel Management protocols/Enema administration

Physical & Occupational Therapist

Licensed therapists will work with you to help you in rehabilitation after orthopedic and other surgeries.

Help with endurance and motor skills lost due to an accident, injury, stroke, illness, or surgery.
Rehabilitation after joint replacement surgery
Work with and help you to get started/relearn ADL skills with
Walker, crutches, cane, rollator walkers, etc,
Range of motion exercises.Home Exercise Program
Strength and mobility programs
Patient and family instructions on assistive device use

Speech Language Pathologists (SLP) Therapists

Our SLP Therapists can assist with:

Assessing cognitive and verbal ability
Instruct in ways to regain and/or improve cognitive and verbal abilities
Assess difficulties in swallowing and aspiration problems in post-stroke patients
Instruct on food intake including pureed, mechanical soft, regular, or liquid diet in swallowing difficulties.

Medical Social Workers

MSW are licensed professionals who offer emotional support, advocacy, and resources to patients with chronic disease or who are recovering from surgery, illness, or injury.
Our home health social worker coordinates with you, your healthcare providers and other members of the home health team so you can receive care that best suits your needs.
The social worker’s role is to provide:

One-on-one education about health and wellness, tailored to patients’ needs, home routines and goals
Counseling for emotional or mental health concerns
Support with coping skills
Advocacy for patients, helping to communicate their needs and any issues that may affect their care to the healthcare team
Help accessing financial assistance and health insurance coverage
Referrals to helpful community resources (e.g., healthy meals, transportation or social outlets like a local senior center)
Help with planning for long-term care
Home Health Aide/Certified Nursing Assistants
Our home health aides are the heart of our home health, helping with personal care, hygiene, and ADLs. Our Aides can also help with therapy-driven range of motion exercises and support short ambulation. Our Aides bring a positive caring attitude to every visit to help you become stronger with each visit.
